**Summary**: This NCECE webinar will explore how patient support needs evolve after an early-stage dementia diagnosis and care planning strategies that effectively engage informal caregivers as key partners. Participants will consider practical approaches to enhancing both patient care and caregiver well-being during the critical period following a dementia diagnosis, and will feature insights from [Dementia Care Aware](https://www.dementiacareaware.org/), a training and support program focused on enhancing dementia care in primary care teams.
