#  Navigating Polypharmacy Management in Older Adults

**Summary**: This National Center for Equitable Care for Elders webinar will focus on optimizing medication plans for community-dwelling older adults, particularly those living with multiple chronic conditions. Participants will review person-centered approaches for shared decision-making in the deprescribing process.

**After completing this session, participants will be able to**:

- Understand medication as a domain of the 4Ms Age-Friendly Health Systems framework
- Explain the importance of health literacy for management of complex medication regimens
- Utilize shared strategies for effective deprescribing and improved medication adherence
