DESCRIPTION:<p><strong>Dates</strong>:&nbsp;</p><p>Session One: October 22, 2024 2pm-3pm ET</p><p>Session Two: November 5, 2024 2pm-3pm ET</p><p>Session Three: November 19, 2024 2pm-3pm ET</p><p>Session Four: December 3, 2024 2pm-3pm ET</p><p><strong>Summary</strong>: This National Center for Equitable Care for Elders learning collaborative will focus on age-friendly approaches to encourage the development and maintenance of healthy lifestyle behaviors in older adults. Health center participants will discuss special considerations for chronic disease self-management in an aging population, including physiological changes and social risk factors.&nbsp;</p><p><strong>Learning Objectives:</strong>&nbsp;</p><p>At the conclusion of this learning collaborative, participants will be able to:&nbsp;</p><ul><li>Understand the impact of improved diet and physical activity on older adult health and quality of life&nbsp;</li><li>Develop patient-centered counseling approaches that prioritize what matters most to an older patient and encourage motivation for proposed lifestyle modifications&nbsp;</li><li>Identify strategies for effective goal setting with older patients that incorporate social supports and community resources</li></ul>
