DESCRIPTION:<p><strong>Summary</strong>: This National Center for Equitable Care for Elders learning collaborative will explore the role of health centers in meeting the complex and evolving needs of community-dwelling adults. Participants will discuss innovative approaches to age-friendly care delivery and cross-sector partnerships that can effectively support this growing patient population.&nbsp;</p><p><strong>Dates</strong>: <span>March 5, 2025, March 19, 2025, April 2, 2025, April 23, 2025 from 1:00-2:00PM ET</span></p><p><strong>At the conclusion of this learning collaborative, participants will be able to</strong>:&nbsp;&nbsp;</p><ul><li><span>Describe the 4Ms of an Age-Friendly Health System: What Matters, Medication, Mentation, and Mobility&nbsp;</span></li><li><span>Apply key principles of person-centered geriatric care to current health center processes and procedures&nbsp;</span></li><li><span>Consider shared strategies for expanding older adult-focused programming and partnering with local organizations to support patients to safely age in place&nbsp;</span></li></ul><p>These sessions are closed for application.
