This series of four webinars will highlight promising practices that improve the prevention, treatment, and management of chronic conditions among health center patients through case studies on health literacy, co-occurring morbidities, patient experience, and food as medicine.</span></p></blockquote><p><span><strong>Session 1 (Wednesday, March 18 at 8 am HT / 11 am PT / &nbsp;2 pm ET)</strong>: Clear Communication, Better Outcomes: Universal Health Literacy Strategies for Chronic Condition Management (NTTAPS: NCECE, CSH, FJ)</span></p><p><strong>Summary</strong>: Effective management of chronic conditions depends on a patient's ability to understand their health needs, navigate the healthcare system, and make informed decisions about their care.
