2020 Diabetes In Special & Vulnerable Populations: A National Learning Series

A logo that reads HRSA Health Center Program beside a house shapeAccording to the most recent Centers for Disease Control and Prevention (CDC) 2017 Diabetes Report Card, the prevalence of diabetes in the United States continues to rise with 30.3 million people (9.4% of U.S. population) having diabetes in 2015. Break it down further and the CDC estimates that 23.1 million people are diagnosed and 7.2 million people are undiagnosed with diabetes.  A study by the American Diabetes Association (ADA) reported the cost of diabetes to be $327 billion for diagnosed diabetes in the United States in 2017.

For Federally Qualified Health Center nationwide, the latest 2018 UDS reports show an estimated 15.1% of health center patients having diabetes, an increase over recent years. Of these approximately 2.4 million plus patients living with diabetes, approximately 33% have uncontrolled diabetes, with HbA1c equal to or above 9% or have had no test in the year. This has remained relatively stable since 2016. In response, the Health Resources and Services Administration (HRSA) created the Health Center Program Diabetes Quality Improvement (QI) initiative in order to: 1) improve diabetes treatment and management; 2) increase diabetes prevention efforts; and 3) reduce health disparities. 

Under the auspices of HRSA and their “Health Center Technical Assistance Partners Strategies to Address the Diabetes Continuum of Care,” the 14 National Cooperative Agreements (NCAs) that comprise the Special and Vulnerable Population Diabetes Task Force are partnering for a third year in a row to provide a multi-part National Learning Series (NLS). The live webinars will build upon last year (2018-2019) to provide health centers, Primary Care Associations (PCAs), Health Center Controlled Networks (HCCNs), and other partners with more in-depth information and enhanced strategies to incorporate into their training and technical assistance initiatives that focus on improving diabetes control for health center patients.
