Diabetes in Special and Vulnerable Populations: Learning Collaboratives

Dates: January - February 2022

The Special and Vulnerable Populations Diabetes Task Force announces the winter 2022 Learning Collaboratives (LC) series. Apply to one of the four LCs to gain strategies, tools, and resources needed to create positive change in diabetes control among health center patients. Choose from topics including “Optimizing Provider & Multidisciplinary Team Interventions”, “Developing Patient-Centered Resources for Diabetes Care”, “Improving Health Systems & Infrastructure Interventions”, and the first collaborative offered completely in Spanish: “Mejora de las intervenciones de infraestructura y sistemas de salud.

Learning Collaborative 3: Improving Diabetes Care and Health Equity in a Changing Healthcare Landscape (in English): NTTAP partners: CSH, Nat'l LGBTQIA+ Health Education, NCECE

SELECTION CRITERIA/AUDIENCE/APPLICATION:

Selection Criteria: Health centers and health center look-alikes will be prioritized for selection. Given our limited capacity per learning collaborative, the Task Force requests that health center organizations indicate ALL of the learning collaborative sections which work for you (based on topic and your availability). We will then notify you about which learning collaborative you have been selected to participate in. 

Audience: Clinical and non-clinical Health Center staff.
