The Special and Vulnerable Populations Diabetes Task Force announces the 2023 Learning Collaboratives (LC) series. Apply (common application is at the end of the module section) to one of the four Learning Collaboratives listed below to gain strategies, tools, and resources needed to create positive change in diabetes control among health center patients. Choose from topics including “Environmental Impacts on Diabetes and Access to Care”, “Creating Your Best Team to Support Chronic Disease Management and Patient Care”, “Improving Emergency Preparedness for Diabetes Management”, or “Understanding Social Determinants of Health and Structural Barriers Affecting Diabetes Care".
