Diabetes in Special & Vulnerable Populations Learning Collaboratives: 2021 Common Application

We invite you to join one of the following four learning collaboratives within the Special and Vulnerable Populations Diabetes Task Force, which build upon the Fall 2020 National Learning Series. Each learning collaborative provides health centers, primary care associations, health center controlled networks, and other partners with in-depth information on improving diabetes control for health center patients during the COVID-19 pandemic.

*Selection Criteria: Health centers and health center look-alikes will be prioritized for selection. Given our limited capacity per learning collaborative, the Task Force requests that health center organizations rank their learning collaborative choices by the topic of most to least interest. You may not be selected for your top choice, however we will make every effort to ensure that each learning collaborative will meet the needs of your organization and patients served.
