Summary: This National Center for Equitable Care for Elders (NCECE) webinar will focus on features and benefits of the Program of All-Inclusive Care for the Elderly (PACE) model within a health center context. The personalized care approach of PACE promotes safe community living for the growing population of older adults, many of whom have a range of complex health needs and varying levels of social support.
At the conclusion of this webinar, participants will be able to:
- Understand the range of medical and social care needs for eligible PACE participants
- Explain the value of integrated care services that focus on older adult health and well-being
- Identify best practices for working with interdisciplinary care teams to successfully enable older adults to remain independent in their communities
