Finding the Sweet Spot: Using Medicare Wellness Visits to Promote Diabetes Prevention in Older Adults

Summary: An individualized approach to diabetes prevention and management in older adults should take into account the 4Ms of Age Friendly Health Systems: Mobility, Mentation, Medications and What Matters Most. Together with individualized lifestyle modifications, clinicians can partner with patients to prevent or manage diabetes, particular for older adults living with multiple chronic conditions. This National Center for Equitable Care for Elders webinar will focus on the value of the Medicare Annual Wellness visit for developing personalized plans to prevent or mitigate diabetes in older adults.

After participating in this webinar, participants will be able to:

1) Describe how to apply the 4Ms of Age Friendly Health Systems (Mobility, Mentation, Medications and What Matters Most) to the care of older adults living with or at risk for diabetes

2) List strategies for individualizing diabetes prevention and management for older patients with multiple chronic health conditions, cognitive, sensory or functional impairments.

3) Identify steps to 'finding the sweet spot' of diabetes prevention and management in older adults, taking into account social determinants of health.
