Improving Rural Health Equity Learning Collaborative

Dates: April 7, 14, 21 and 28, 2022 (Thursdays from 1:00-2:00PM ET)

Summary: Rural-dwelling older adults, including former agricultural workers and their families, often experience health disparities due to more limited access to preventative healthcare, proper nutrition, and transportation services. This National Center for Equitable Care for Elders (NCECE) and Migrant Clinicians Network (MCN) joint learning collaborative will discuss strategies to enable patients in rural areas to successfully age in community by identifying and addressing their social determinants of health needs.

After participating in this webinar series, participants will be able to:

  • Understand risk factors that impact the well-being of rural residents as they age
  • Examine the impact of social determinants of health (SDOH) needs on well-being and overall life expectancy
  • Develop strategies for leveraging local strengths and resources through community partnerships

Intended Audience: Health center providers and outreach staff who care for older adults, including former agricultural workers and their families.
