Motivating Healthy Habits in Older Adults Learning Collaborative

Summary: Healthy lifestyle habits related to diet and exercise play an important role in preventing and managing chronic conditions such as diabetes, but older adults may face unique barriers to adopting or modifying these habits. This learning collaborative will highlight the importance of identifying age-appropriate motivations for behavior change and setting realistic goals. This activity will be co-facilitated with the National Center for Health in Public Housing (NCHPH). 

Learning Objectives

At the conclusion of this learning collaborative, participants will be able to:

  • Understand the impact of improved diet and physical activity on older adult health and quality of life
  • Develop patient-centered counseling approaches tailored for older adults who are uncertain about proposed lifestyle modifications
  • Identify strategies for effective goal setting with older adult patients that incorporate social supports and community resources

Intended audience: Health center providers and staff who care for older adults and residents of public housing
