This learning collaborative focused on building critical skills in communication with patients who have a serious illness and developing strategies to meet their unmet palliative needs, with an emphasis on dementia care.

  • Understand the role of primary palliative care and implement concepts into their health centers
  • Apply techniques needed to discuss serious news and goals of care with patients and their families
  • Develop strategies for providing continuous and quality care for their older adult health center patients in the beginning stages of palliative care
