Session Objectives
1. To understand the impact of silo’ed care on older adults.
2. To learn about models of integrated provider training with specific focus on primary care and oral and behavioral health integration for older adults with chronic, co-morbid conditions.
3. To learn about models of integrated care delivery with specific focus on primary care and oral and behavioral health integration for older adults with chronic, co-morbid conditions including exploration of the patient-centered medical home (PCMH).

4. To share ideas and best practices of integrated training and care delivery for meeting the needs of older adults with chronic, co-morbid conditions.
