Webinar Series: Caring for an Aging Population

Summary: This webinar series will focus on increasing the capacity of health centers to meet the health and social needs of older community-dwelling adults followed in primary care.   

Unprecedented growth in older populations and increasing awareness of social determinants of health presents challenges and opportunities for health centers focused on providing optimal care for the patient population across the lifespan. 

After participating in this webinar series, participants will be able to

  • Understand the impact of the growing number of patients approaching or over the age of 65 
  • Explain the value of integrated and highly coordinated care for populations of older adults, especially those living with multiple chronic conditions or functional limitations 
  • Identify and integrate age-friendly strategies to reduce harm and optimize the health and well-being of older adult patients
