Improving Community-Based Geriatric Care

Summary: This National Center for Equitable Care for Elders learning collaborative will explore the role of health centers in meeting the complex and evolving needs of community-dwelling adults. Participants will discuss innovative approaches to age-friendly care delivery and cross-sector partnerships that can effectively support this growing patient population.
Dates: March 5, 2025, March 19, 2025, April 2, 2025, April 23, 2025 from 1:00-2:00PM ET
At the conclusion of this learning collaborative, participants will be able to:
- Describe the 4Ms of an Age-Friendly Health System: What Matters, Medication, Mentation, and Mobility
- Apply key principles of person-centered geriatric care to current health center processes and procedures
- Consider shared strategies for expanding older adult-focused programming and partnering with local organizations to support patients to safely age in place
