Summary: This National Center for Equitable Care for Elders learning collaborative will focus on age-friendly approaches to encourage the development and maintenance of healthy lifestyle behaviors in older adults. Health center participants will discuss special considerations for chronic disease self-management in an aging population, including physiological changes and social risk factors.
Learning Objectives:
At the conclusion of this learning collaborative, participants will be able to:
- Understand the impact of improved diet and physical activity on older adult health and quality of life
- Develop patient-centered counseling approaches that prioritize what matters most to an older patient and encourage motivation for proposed lifestyle modifications
- Identify strategies for effective goal setting with older patients that incorporate social supports and community resources
