Building Capacity for Care Coordination Through PACE Learning Collaborative

Wednesdays from 1:00-2:00PM ET: April 7, 14, 21 and 28, 2021

Abstract: Programs of All-Inclusive Care for the Elderly (PACE) promote safe community living for the growing population of older adults, many of whom have complex health needs and varying levels of social support. This learning collaborative will discuss the features and benefits of this model within a health center context and explain the PACE provider application process.

Learning Objectives: At the conclusion of this learning collaborative, participants will be able to:

  • Understand the range of care needs for eligible program participants
  • Examine organizational requirements to provide PACE health care services
  • Develop plans to assess feasibility of health center PACE implementation
