Population Health Management Task Force National Learning Series

Summary: This March, thirteen National Training and Technical Assistance Partners (NTTAPs) host a four-part webinar series for health centers serving priority populations, including: residents of public housing, agricultural workers, patients experiencing homelessness, and other at-risk populations including older adults, and Asian American, Pacific Islander, and Native Hawaiian patients. This webinar series will take a deeper dive into issues around systemic, structural, and environmental barriers and strategies for improving chronic disease management and care. Each webinar will focus on specific strategies for developing, evaluating, and supporting effective health care delivery models designed. Coordinators will ensure that webinar materials are practical, providing information and enhancing skills directly related to the training recipients.

March 27: Team-Based Approaches for Managing Complex Health Needs: Join the Migrant Clinicians Network, the National Center for Equitable Care for Elders, the National Network for Oral Health Access, and the National Nurse-Led Care Consortium for the closing session of the Population Health Management Task Force’s National Learning Series. This webinar will focus on innovative and collaborative approaches for health centers as they provide comprehensive care to individuals living with multiple chronic conditions.   • Understand the impact of fragmented care on patients living with multiple chronic conditions (MCC)
  • Identify key elements of comprehensive MCC interventions that prioritize unique patient needs and preferences
  • Consider innovative strategies to improve care coordination and interprofessional collaboration
