Summary: This National Center for Equitable Care for Elders learning collaborative will highlight the value of partnerships with aging services network agencies to address the support needs of community-dwelling older adults. Health center participants will discuss models of collaboration that close related gaps in care and reduce health disparities experienced by older patients.
At the conclusion of this learning collaborative, participants will be able to:
- Explain the impact of community supports on the ability of older adults to safely age in place
- Identify local needs and available home- and community-based services for older adults
- Utilize strategies for developing, assessing, and maintaining successful relationships between health centers and aging organizations
