Motivating Healthy Habits in Older Adults

Summary: This National Center for Equitable Care for Elders learning collaborative will highlight the importance of working with older adults to set realistic goals for chronic disease management. Participants will review special considerations for this patient population when adopting or modifying lifestyle behaviors related to diet and exercise.

Learning Objectives

At the conclusion of this learning collaborative, participants will be able to:

  • Understand the impact of improved diet and physical activity on older adult health and quality of life
  • Develop patient-centered counseling approaches tailored for older adults who are uncertain about proposed lifestyle modifications
  • Identify strategies for effective goal setting with older adult patients that incorporate social supports and community resources
