Promoting Age-friendly Environments: Critical Determinants of Health

Dates/times: Feb, 2, 9,16 and 23, 2023 (Thursdays), 1pm-2pm EST

Summary: This National Center for Equitable Care for Elders learning collaborative will highlight the importance of accessibility and inclusion in supporting older adults to safely age in place. Participants will learn to identify, and if possible, address some of the physical and social barriers to healthy aging. 

We will provide examples of best-practices of successful age-friendly community initiatives as a starting point for participants to identify social and environmental needs of their own patient populations. We will also emphasize inclusive-design across the continuum of health, well-being, and care of aging populations through partnerships with local community-based organizations.

  • Understand community characteristics that support well-being across the lifespan 
  • Utilize environmental scans to identify barriers to developing age-friendly initiatives 
  • Develop approaches to build and strengthen community partnerships addressing social determinants of healthy aging
